    I’m missing some important references like the one of New Zealand Guidelines Group. Management of burns and scalds in primary care. Wellington (NZ): Accident Compensation Corporation ; 2007:1-116, and the publication of the Education Committee of the Australian and New Zealand Burn Association. Emergency Management of Severe Burns (EMSB) After cooling, it is indeed recommended to cover the burns clean and cover the victim warmly. Certainly clean plastic household foil can be used to cover the wound, applied in layers and is not circularly wrapped in an extremity due to a risk of seizing hazard in edema formation. Covering the wound reduces pain and can be used prior to further professional medical help. Furthermore, it is still important not to apply a cream or other topical means because it can harm the wound inspection at the ER and/or burns centre (of course only in the case of severe burns). Seems to me important enough to mention.

    There appears to be some conflicting information on coconut water and egg white in the Justification and Evidence to Decision Framework Highlights bullets: Coconut Water - We no longer recommend coconut water as a storage solution as recent studies provide conflicting evidence; nor do we recommend egg white, since the beneficial effect was not confirmed by new studies. - A recommendation was not made for Eagle’s Medium (aMEM), aloe vera or coconut water because of inconclusive or conflicting evidence. Is coconut water not recommended, or is there not a recommendation for or against it? Egg White - We no longer recommend coconut water as a storage solution as recent studies provide conflicting evidence; nor do we recommend egg white, since the beneficial effect was not confirmed by new studies. - There is insufficient evidence to recommend for or against temporary storage of an avulsed tooth in probiotic media, Epigallocatechin-3-Gallate, Dentosafe® box, Tooth rescue box, or egg white compared with cow’s milk. Is egg white not recommended, or is there not a recommendation for or against it?

    The current pre-course assessment for ACLS allows for multiple attempts. So for comparison, a trial of single attempt pre-course evaluation vs post evaluation may be warranted.

    I had published a study detailing an Instructor led training of motivated laypersons for Pediatric CPR training for a certificate course. “A comparative study of pediatric basic life support course for motivated laypersons and health-care personnel “ in Journal of marine medical society. To my mind, such courses ,which train the motivated layperson ,should be offered by CPR training centres on a regular basis with an intent to train such an individual( of any age) for cardiac arrest and choking related event. A 10 hour pre-course was utilised by our team to prepare the laypersons for such a certification course.

    Thank you for the time you have taken to read and comment on our draft CoSTR on compression wrap for closed extremity joint injuries. The paper by van den Bekerom MP et al in the J Athl Train (2012) is a literature search and systematic review of evidence up to 2010. The ILCOR GRADE process excludes systematic reviews, however they are used as part of the reference inclusion check to ensure that the ILCOR search strategy is inclusive of all know literature on the topic.

    Thank you for the time you have taken to read and comment on our draft CoSTR on compression wrap for closed extremity joint injuries. The CoSTR review process generates the published scientific information and derives a treatment recommendation based on the GRADE analysis of this evidence. In this PICOST the evidence was equivocal. As can be seen in the Justification and Evidence to Decision Framework Highlights, the Task Force debated the possibility of raising the level of recommendation to a more definitive conclusion but decided that there was insufficient evidence to support either the intervention or the comparator.

    Many thanks for this review. In some countries, such as Sweden, there are legal restrictions on administration of drugs by non-professional individuals, a recommendations such as ”administration of aspirin as first aid treatment” instead of ”by first aid providers” would help our guidelines writing, how does ILCOR see such rephrasing?

    The Task Force thanks you for your comment. As highlighted in the review, the critical time window remains unknown, but within the review we used a cutoff of 2 hours from onset of pain. Regarding the description of traumatic and non-traumatic, we agree that such description would be helpful to include in educational material. The Consensus on Science and Treatment Recommendation (CoSTR) provides a detailed review of the science and a treatment recommendation based on this science. The next stage in the process is for interested national and international bodies to use the CoSTR to develop detailed clinical guidelines and educational materials for wider dissemination.
